Integrated Community Options (“ICO,” “we,” “us,” or “our”) is committed to protecting the privacy and confidentiality of the individuals we serve, their families and representatives, our website visitors, and our staff. This Privacy Policy explains how we collect, use, disclose, and safeguard information across ICO and its agencies — Integrated Community Options North (ICON), our multicultural adult day program and career services agency, and Pasa Alta Manor (PAM), our 24-hour residential care home for adults with developmental disabilities. It applies to information collected through our website, our referral and intake process, and in the course of providing services.
  1. Scope

    This Policy applies to ICO and all of its agencies, including ICON and PAM. Where an individual agency has a practice that differs from this general Policy, that agency's admission agreement or program-specific notice will control.
  2. Information We Collect

    Contact and inquiry information: name, phone number, email address, role, Regional Center affiliation, and any message submitted through our website's contact, referral, or intake forms.

    Referral and intake information: information provided by Regional Centers, family members, conservators, or other referral sources to determine eligibility and program fit.

    Consumer and participant records: information created or maintained while providing day program or residential services — for example, person-centered plans, behavioral support information, health and medication information, incident reports, progress notes, and emergency contacts — consistent with California community care licensing and Department of Developmental Services recordkeeping requirements.

    Employment information: application materials and related information from job applicants and staff.

    Website and technical information: IP address, browser and device type, and general usage data collected automatically when you visit our website.
  3. How We Use Information

    Evaluate eligibility and coordinate referrals with Regional Centers
    Deliver, coordinate, and document day program and residential services
    Communicate with participants, families, conservators, and Regional Center service coordinators
    Meet licensing, funding, and regulatory reporting obligations
    Support the health, safety, and rights of the individuals we serve
    Improve our programs, facilities, and website
    Respond to inquiries submitted through our website
  4. Confidentiality of Consumer Records

    Records concerning individuals referred by a California Regional Center are confidential under the Lanterman Developmental Disabilities Services Act (Welfare & Institutions Code § 4514) and related Department of Developmental Services regulations (Title 17, California Code of Regulations). We disclose such records only as permitted or required by law — including with the written authorization of the consumer, their conservator, or legal representative — or to the individual's Regional Center, physicians, or other professionals directly involved in their care.
  5. Health Information

    To the extent we create, receive, or maintain health information protected under the Health Insurance Portability and Accountability Act (HIPAA) or California's Confidentiality of Medical Information Act (CMIA), we handle that information consistent with those laws, including safeguards on use and disclosure and, where applicable, an individual's right to access their own records.
  6. How We Share Information

    We do not sell personal information. We may share information with:

    The individual's Regional Center and assigned service coordinator

    Licensing and regulatory agencies, such as the California Department of Social Services and the Department of Developmental Services

    Healthcare providers, therapists, and other professionals involved in a participant's care, with appropriate authorization

    Emergency responders and law enforcement, when necessary to protect health or safety, or as required by law — including mandated reporting of suspected abuse or neglect of a dependent adult or elder under California law

    Vendors who support our operations (for example, IT or payroll providers), who are bound by confidentiality obligations

    As required by subpoena, court order, or other legal process
  7. Data Security

    We maintain administrative, physical, and technical safeguards designed to protect personal and consumer information against unauthorized access, use, or disclosure, appropriate to the sensitivity of the information involved.
  8. Your Choice and Rights

    Individuals we serve, and their conservators or legal representatives, may request access to or correction of their records, consistent with applicable law and our recordkeeping obligations. Website visitors may contact us to ask what contact information we hold about them or to opt out of further outreach. As a nonprofit organization, ICO is generally not subject to the California Consumer Privacy Act (CCPA); we nonetheless aim to honor the spirit of these rights where practicable.

  10. Data Retention

    We retain consumer and participant records for the period required by applicable community care licensing regulations, funding agreements, and other legal requirements, after which records are securely destroyed or de-identified.

    ICO's residential and day programs serve adults. We do not knowingly collect personal information from children through our website.

    We may update these Terms from time to time. The “Last Updated” date above reflects the most recent revision. Continued use of our website after changes constitutes acceptance of the updated Terms.
